Ambulance Trust Investigates Staff Access to Records of Man Killed by Tiger

Yorkshire Ambulance Service has reported the matter to the Information Commissioner's Office. The family has been informed

Yorkshire Ambulance Service NHS Trust has opened an investigation after two members of staff allegedly accessed, or tried to access, information about William Chapman, the man killed by a tiger at Yorkshire Wildlife Park, without a valid reason. LBC reported the investigation on 16 October.

The trust found two separate incidents involving two staff members. A spokesperson said there "may have been two instances" of access or attempted access to incident or medical information "without a legitimate reason to do so". The concern emerged from proactive audits. The trust has told Chapman's family, has started an investigation into "the actions of these individuals" and has reported the matter to the Information Commissioner's Office. The investigation is continuing.

Chapman was mauled by a 30-stone Amur tiger at the park near Doncaster on Saturday 10 October. LBC gives his age as 24 in its text and 23 in a photo caption. The inquest opened on Thursday and was adjourned until 18 March, with the provisional cause of death listed as multiple traumatic injuries and animal interaction. His family described him as a "bright young man".

The park reopened to visitors on Friday, nearly a week after the attack.
